You probably have an osseous prominence on the top of your foot. Normally, the laces cross over the top of your foot and sort of make a series of "X"s where they cross. Feel for the tender area, and rather than having them cross, go one hole where the laces are parallel to the opening at the top of the shoe. If this is unclear, I can post a pic to illustrate.
Richard, to keep it looser by the toes, get them the way you want them, tie a knot a few holes from the toe of the shoe, lace the rest normally. That way, you can get the top tight without crowding your toes.
