I was with my dad when the doc did a preliminary mental acuity test on him. I remember that she said a word and told him to remember it. Then two or three minutes of questions. She asked him to fold a piece of paper four times. My dad did his typical exacting thing - all four parts of the paper were precisely the same size, he made excruciatingly precise, tight folds, and the result was perfect. He took so long I thought she would mark it against him, but that is the way he's been all his life (when he digs a ditch it is a work of art). After about five minutes she asked him the word - he recalled it with no problem.
But I still had to go get him a couple of days later when a LEO in the next county called and said my dad was lost.
After talking to some people I'm pretty sure my problem is stress and the resulting lack of rest.
