Seriously, if you can get the truck near the pour grab one friend, buy some 2x4's with some 2x2 wood stakes, form the 10x12, cross check for square, add some dirt/stone around the outside to keep the forms from lifting, cut your 2x2's level with the top of the form, make an "L" shape screed board, have two garden rakes on hand, pour, rake, screed, then broom and call it a day. A 10x12 is a pretty easy novice pour.
Option B: Go to Home Depot and talk to the "contractors" out in the parking lot looking for small jobs.
