I wasn't in the aircraft when it happened, but my boss knows firsthand what happens when you try to toss something from an OH-58 and it won't clear the tail rotor. He's a lucky man for two reasons.....he didn't get killed and he made O-6!

Can you find the cartoon of the forward flight airflow over the fuselage? That is the one that will carry the flag into the tail rotor.

If you are in a decent, and if you are uncoordinated sliding right and if you really try hard enough you can get the flag into the tail. That is a lot of trying though. Of course it is not very smart anyway as he is not doing much flying assuming he is the PIC.

If you watch the second youtube video at the very end it shows the induced flow going right past the end of the tail boom (where the TR is). That video shows a prototype Apache with the TR on the top of the vertical stabilizer. All production Apaches have the TRs mounted low on the tailboom. At 150 knots your chances are very good of sending a flag or whatever else into the TR.

I didn't stay at a Holiday Inn express last night I happen to be a commercial helicopter pilot and do this for a living.

Why?

The shuttle was never designed for interplanetary travel. As much as some people want to believe, the shuttle never "represented mans reach for the stars". It was just a delivery truck.
