That gas turbine accident video sounds like the crane operator let the brake go from the sound of the cables? We had that happen at NASSCO shipyard when the crane was moving workers from the pier onto the USS Sacramento (oiler) which was in the floating dry dock and a real pain to walk up all those stairs! Myself and the other government watchdog got called at 0130 to hurry and come in to watch how the shipyard would handle the deaths of 3 men and serious injuries to 7 others.
When we got up on the main deck the basket was still on it's side and a pile of grey stuff next to it, the other guy told me that was someone's brains as one guy had their head crushed about flat!!
