Non-healing or open wounds are treated with a device called a wound vac. That is the (bodily) fluid reservoir connected to a vacuum generator. Diabetics and IV drug addicts are especially susceptible to developing wounds requiring this therapy. That this was laying in the street suggests someone "discontinued" their treatment right there. Would imagine a sad story is associated.
