Rear seat access has not been an issue. I put Trail Gear rock rails on it that help with access and add a tough look to the truck. My 3 y/o gets in and out w/o a problem and we can reach his seat without reaching. We get 17 MPG driving 100% city driving. And my highway milage has been in the 20-24 MPG range depending on how much crap I have packed into it. I also use a rear hitch deck that adds a bit of cargo capacity to it.
