My only suggestion at this point is a question about format and eliminating the 3-5 month delay at the prnting company ... why not publish it in PDF format on a CD-ROM? Probably 80% of the people out there can already read PDF files and print them, and for any that don't have the Adobe reader, it could be included free on the CD, just as it is on many magazine 'freebee' CDs.
The only problem would be those without computers, and you could always print copies and put it in a binder, loose-leaf format like Mr. Pellow did with the early 'Maestro' books!
