I sure hope that this is a hoax. That does look like a serious injury, possibly caused by a chain saw or some other tool. A lot of things can cause really bad injuries. One thing that woke me up was the number of garage door installers that were killed on the job. I was looking into insurance requirements for that business. More than a few of the deaths involved the springs, as you can imagine. The wind-up spring on a sectional door can get away from you and do big time damage as seen in that pic.

But, a drill motor up the nostril with I don't care what kind of attachment, would not seem to do what I see there. And, to tell you the truth, I wish I hadn't seen any of it. I lost a little respect for you on that one. I repeat, I hope that is a hoax or a lie.

Ok this sounds a little grisly but that stuff on the side of his face is blood, not torn away skin. It is just blood going down from his nose.
