NASA Study On Aviation Safety - Won't Release
This is kind of bizzare. NASA conducts one of the largest surveys of pilots on aviation safety incidents, collects 20,000 responses over 5 years, then refuses to release the results because

They spent taxpayers' money to conduct the survey. The results of the survey do not belong to NASA, but to the public.

IIRC, a FOIA request was made quite a while ago and NASA has not complied with the request for a variety of reasons.
