Anyone recognize this plug

I've never seen an outlet for a plug like that. Maybe it's DC.

How did you add another spade. Did you have to strip the cord the entire way and add in a wire too, then tape it all back up? What about the other end? Or do just ground to the floor, or couch with a sheet metal screw?
