How did a head stud washer end up here? Broken head studs?

Was recently doing an inspection underneath my 1983 911 SC 3.0L & I came across what appears to be a head stud washer falling out of place. I was initially inspecting the underside to pinpoint an exhaust leak but it ended up being compression loss between the cylinder & the head. It is making a popping noise indicative of multiple broken head studs. With that being said, is it possible for a head stud washer to fall so out of place that it ended up in this spot? Its right above one of the oil return tubes closest to cylinder 4.
