clunking window

My drivers side window has been making a clunking sound when i lower the window.
It's also not lowering smooth, if free falls the first 2 inches or so.

So i decided to remove the door panel to see what was going on in there & this is what i found.

(2) missing bolts and the remaining (3) bolts only holding by 1 turn of the threads. The missing bolts fell out onto the ground as i removed the panel.

There was also an allen head bolt that was hanging in the bracket that didn't appear to have anything to thread into, any thoughts as to where this goes?, see the pic below.

Then i noticed that the visqueen was held in place with masking tape, and a pair of wires on the outside. Well that got me interested in what was happening in there, so i peeled the tape back & saw this actuator that is held in place by plumbers tape - nice mod by the PO.

After reinstalling the bolts that fell out, the window now is as smooth as butter.
no more clunking and the rattle that's been there is now gone too
