Check fuel pressure. Most of the other problems would cause codes but most of the time low fuel pressure won’t.
Pull a vacuum line off and spray carb cleaner in it while starting and see if it makes a difference.
How many miles on it?
Try unplugging the maf while it’s running and see if it changes.
