He's talking about prophylactic vaccines... which is what the polio and smallpox vaccines would be. He's saying that using these when the virus is spreading rapidly, which has been the case for both polio and smallpox, will pressure the virus and cause it mutate and that ultimately this will result in immune escape. It makes no logical sense to say that a vaccine that generates an immune response works in one instance but not in another instance and that in the other instance it's actually making matters worse.
