I am not sure if they remedied it by then, but the cooling systems on the earlier E39's were pretty weak, with a major liability being the water pump with the plastic impeller. New ones have a metal impeller, and are worth installing. Other than that, the cars are basically bulletproof with many I see lasting 200K without major issues. Don't know about the manual tranny.
