It looks like the linkage is hitting the cross member at the top... preventing it from moving towards the rear of the car, thus no 2nd, 4th, or Rev.
Try moving the linkage down to the second hole, (will shorten the throw), and move the linkage away from the cross member.
Also, turn the plate around, so the bolt on the bottom mates to the flat section of the shift rod on the transaxle. (This may be slipping in its current configuration)
