Check you door lock mechanism. Mine did the same and it ended up being the door lock even though I suspected the top. Try a little wd40. Best solution would be to gain access and use grease.

How far do you insert the screwdriver when you move the tab washer?
I deformed my old ones like that checking the movement. They are quite soft and easy to bend. Be careful only to move the washer.

Same here. The special wrench work with the 12mm brass nuts, but my installation kit came with 13mm steel. Hmmm.
