One of the biggest design flaws that I can recall was in a high-rise building in Manhattan that had a sizable chance of coming down because of wind loads. If I remember correctly, a grad student first discovered the problem, which was confirmed by the original engineers and the building was quietly retrofitted (quickly, too) without the public being advised of the potential problem.

Nope. I would think that they'd be more prevalent due to poor materials and corruption (Bob's company got the job because he's a friend/cousin of a friend or bribed the right people) and less concern for litigation or repurcussions. But maybe not.

Great but how do you make a piece of glass that big and heavy?

I've an idea they might float the molten glass on another fluid as it cools.
