this example is telling on themselves, its incompetence masquerading as a technical challenge.

there was a suggestion of a rocket example ... its closer to this than challenger:

Something I realized this morning (very late and maybe discussed already): I don't remember seeing anything about non-destructive examination of the mini sub during construction. Did I miss that? Or did they just not do it?

It sounds like they had some sort of acoustic emission monitoring system on the carbon fiber hull but with everything else on the sub being substandard, I expect their system wasn't calibrated properly and/or wasn't tracking history.

This was the part of the issue with David Lockridge, the guy they fired after he raised safety concerns. He wanted to do NDT but apparently Stockton Rush would have none of it claiming that no NDT equipment would work on 5” thick carbon fibre…

I got the impression that it was done mostly "by feel"

With every dive cycle it made less noise and they convinced themselves once it was not making any more noise it was "broken in" and since it didn't implode it was safe.

I think that they convinced themselves it was safe because they said so… a lot. The more times they said it, the safer it became.

It's my understanding that they did in fact have acoustic emission monitoring. We use it on high energy piping at work. If it's not calibrated correctly and if you don't record, track and analyze the data it's pointless or worse, gives you a false sense of security.
