They are tough to make and to get them to mesh right. Takes some great skills to make well - even with modern CNC machines.

Cut spiral gears one way.
Cut spiral gears the other way.
Match up keys narrow or wide so it all looks like fishy bones.
Repeat smaller or larger other side. With flex spacers to absorb vibrations.
