When I mig welded trailers together for a living, welds that looked like that one apparently does (its just one view in a picture) were discouraged and sometimes outright rejected. I had to keep the heat closer to the elements to be joined and use less fill for such a weld. But I only had one boss and only had one welding job, so what do I know. It may just have been him.

Cleanliness, and I think sometimes it's from the mig wire.

They can't be can't be completely avoided, but in some applications they are stress points that lead to cracking and have to be removed.
