Believe it or not that is the stuff we are made of.
Emergency Room staff see that..and much much worse..almost every day or hour sometimes.
The venerable Mr Tobra not excluded from this. He has seen some sheet.

The cut line looks smooth so there might have been a medical reason such as removing a tumor, if this had happened on purpose.
If not on purpose, then it might be somebody hopping a barbed wire fence they were not supposed to hop, in which case our opinions of the patient might change.
