A few times I have read or heard accounts of a driver losing control and braking as hard as they could then "we hit the grass (or ice, or any other low co-efficient of friction surface) and it sped up"

I'm sorry. This is physically impossible. The rate of deceleration may have approached zero, but there was no force applied to the vehicle to accelerate it.

Hitting the accelerator in error rather than the brake is probably more common than people realise. We know it happens in low speed incidents (Youtube is full of examples), why wouldn't it happen in high speed incidents? (Just more likely to go unrecognised)

People who are old or disabled who insist on walking 3 abreast in a corridor at a pace that would make a paralytic sloth look like Usain Bolt, then give you the evil eye when you push past them on the way to an emergency.
(Dude, it's a hospital, sometimes doctors rush)

Similarly, drivers will often turn into a business and STOP, oblivious to anyone behind them still in the road trying also to pull into the business (like a gas station, for example).

I've actually tried to educate those people. It's amazing how totally unaware they are.

The term I have always used is "bottlenecking".

Happens everywhere but an example that especially hits close to home for me is when I am visiting a property on business (landscaping) and there is PLENTY of parking space but......someone is bottlenecking the front part of the parking area.....typically all they would have to do is PULL UP a bit!

LOUD PEOPLE!

(yelling to make a point!)

Especially those who choose to talk on their damn cell phones while in public. A lady who lives behind me is always on her cell phone. Everytime she drives by she is on it...and when she walks her dog she is on it. I'm waiting for the day when the dog stands up on his back legs...snatches the phone away from her...and say WTF?

And you can always hear her coming a mile away....sheeesh!
