Based on what I can see in the picture and what I know from playing with turbos, the flange (top arrow in your picture) is prolly the oil drain and needs to be at the bottom, or within a few degrees from bottom. the oil feed should be 180 from the flange and is threaded for a fitting. It looks like you have a garrett? turbo, and if so the side taps are for water cooling and should be plugged in an air cooled application.

Not that I'm a guru but that looks like watercooled turbo. Top arrow looks like oil-drain and bottom arrow is water input or output. Looks like custom turbo from some passenger car with that 90-degree elbow just behind the turbine...which car is thing going into?

I'm going to put it on the 318i bmw. I just got the exhaust manifold and the FMU. Still need bigger injectors. I'm going to run it without an intercooler at first and then add one later.
So where is the return hole? |

It doesn't matter which way you push water trough it...oil input is top one on second picture and oil-drain is top on first picture.

P.S. Vert¡cal line going trough oil flanges shouldn't tilt more than 15 deg. from vertical axle in any direction, according to Garrett specs.

If you can run a water line to the turbo, it's a cooler running setup.

If it's going into a BMW, by all means hook up water to it.
If it was going in a 911 then you would plug the water ports. Oil in is the smaller round hole, oil out is the larger square hole. |
